[PLEASE READ] - I understand that after submitting this request, an email with my consent forms will be sent to me shortly. These must be completed at least 48 hours (2 days) before my appointment. If I don’t complete them, my appointment will be canceled.

If I don’t complete my paperwork before the deadline and miss my appointment, the clinic will decline to establish care and will not reschedule. Cancellation fees may apply.
